Interact with each consumer in a variety of potential settings, including community homes, family and individual homes and generic community settings, providing personal care, instruction, guidance, mentoring and companionship, according to the individual's Person Centered Plan/Individual Support Plan (ISP). The DSP provides a level of care that enhances the health, safety and contentment of every consumer served. *This position will provide services in Brookville, Punxsutawney, Reynoldsville, and DuBois, PA. *This position will also be expected to fill in as a substitute on other cases as needed. High School Diploma or G.E.D. PA State Criminal Clearance current within 1 year. PA State Child Abuse Clearance (when consumer is under age 18)current within 1 year. Valid Driver's License and reliable transportation.
